Root cause analysis (RCA) is a systematic process for identifying “root causes” of problems or events and an approach for responding to them.   • Decision Analysis (DA) provides a logical framework for structuring and evaluating a decision scenario, with an end goal of clarity of action. This framework involves characterizing uncertain events, formulating creative alternatives, and incorporating decision-makers preferences and values. Students are introduced to a set of coherent tools used for framing problems and performing logical analysis, including decision modeling in Excel. Topics include decision trees, sensitivity analysis, influence diagrams, value of information, risk preferences and Monte Carlo simulation.
